The real problem in error management is the human emotional element and how to simultaneously address the error, coach the individual, and modify the system infrastructure to avoid recurrence. Quantum leaders should engage in all but one of these behaviors to achieve positive outcomes in addressing errors:

a. Avoid knee–jerk responses to create an impact and perception of doing something.

b. Establish an infrastructure with resources prepared to respond to errors in a comprehensive, timely manner.

c. Engage risk management staff to assess the negative impact on the organization.

d. Place all involved persons on administrative leave until the error is investigated.
