Your fact sheet should include the following sections: (a) general overview of what the resource is, (b) description of the supports and services/programs provided, (c) description of who this resource is appropriate for (e.g. students with intellectual disability), (d) who is eligible for services and when, (e) what the services cost or how they are funded, (f) description of the referral process and required documents (if needed), and (g) how and when do they become involved in the transition process. 2. Personal thinking a. You will also submit an individual, typed thinking on the mapping process. Your thinking should involve the following sections: i. Method- Describe how you sought to learn about the community resource (e.g., searching the web, phone interview, in-person interview of staff member, interview a stakeholder about their experience with the organization or agency, other). You must use a minimum of two research activities to learn about the community resource (e.g., phone interview and website; visit to the agency and phone interview). You should identify the questions you want to ask ahead of time for your visit, phone interview, etc, and you must submit a list of the interview questions (pre-determined) that you asked. The list of questions can be embedded into your description of your method or submitted as a separate list. ii. thinking-t hinking on the experience. Your reflection might address questions such as: What was the mapping process like? What did you expect to learn about the community resource, and did your results align or differ from your expectations? How will this experience and the knowledge you have gained impact you as a teacher? How do you think this process would go for parents?
