1. Locate the policies and procedures. List the policies relevant to preparing for and assessing clients who have varied needs, for example: mental health needs, cultural care needs, activities of daily living needs etc.

2. Locate and document the types of assessment tools / processes used in the service

Skills to be demonstrated while on work placement. All documentation is to be submitted with your work placement book:

1. While engaging with clients, visually observe and evaluate the needs of three (3) clients. From your observations:

a. Identify the specific needs of each client

b. Document how you will respond to the specific needs of each client by way of analysing and using

critical (particular) thinking skills

c. In addressing the needs of the client, identify how you will focus on and improve the areas they already have strengths in

d. Using the information collected, provide an analysis (summary) of the clients co-existing needs

and use the evidence as a base for your judgements on their needs

e. Discuss information collected for points a), b), c), d) with your direct supervisor for their

feedback, ensuring to document your observations and the feedback from your supervisor
