John is a tenured employee and in the last ten (10) years, he has received satisfactory, if not excellent ratings on his performance reviews. He has perfect attendance and is a very helpful employee. John never says "no" to any requests by his supervisors and always volunteers.

In the last six (6) months, it was discovered that John has been failed three (3) Compliance audits and has sent a document with incorrect information. Auditors discovered the errors, and the company was cited and fined. Furthermore, John witnessed a safety issue, but didn't tell anyone about it. The employee who he witnessed was injured.

What would you recommend on a course of action? What considerations would you have?
