All names have been changed and locations are generalized to maintain confidentiality. 'Jared' is a white seven year old boy in the second grade and lives primarily with his mother due to his parents being divorced. He lives in a middle class area of town and his mother is able to afford after school care for him. He will occasionally visit his father on the weekends. While at school, 'Jared' has difficulty listening to his teacher and completing work quietly at his desk. He will often get up or move around in his seat, and sometimes asks to go to the bathroom without needing to do so to run down the hallway and back. Jared's favorite parts of the school day happen at gym and recess where he is free to run and throw balls in a game. After school he goes to an afterschool day care while his mother finishes work. He often will get in trouble for running away from his group when there is not a physically active game available and will try to find any ball or throwable toy to begin throwing it across the room. He will interrupt conversations with leaders whenever something trivial happens around him and causes his attention to be drawn to it. Jared gets sent to the office while at after school care around two times a week, and will have uncontrollable episodes where he continuously runs or climbs indoor play structures to escape his leaders about two times in a month. His mother will often be called when one of these episodes happens and will leave work early to pick him up. She also reports that he has similar difficulties at home and when he visits his father. OPEN D Share N 3 ... 7 Jared fulfills a Predominantly Hyperactive-Impulsive Presentation of ADHD as defined by the DSM-5 (CDC, 2023). Jared does struggle with inattention, and struggles with listening, becoming distracted and dislikes sit down work at school, but he does not have any problems with forgetfulness. He fulfills five criteria laid out for Inattention, but six criteria are needed for him to be diagnosed with ADHD (CDC, 2023). Jared does fulfill the criteria for ADHD with Hyperactivity and Impulsivity, as he struggles most with sitting still, interrupting his leaders and is almost always on the go. Jared fulfills seven of the DSM-5 criteria for Hyperactivity and Impulsivity ADHD and his symptoms have been present for at least the past six months (CDC, 2023. His struggles have been recorded at school, home and his afterschool care while interfering with his functioning at school and afterschool. ^
