A single IV bolus injection containing 140 mg of gentamicin is given to an adult female patient (63 years, 55 kg, 5 foot 1) for a sepsis due to E.coli which has been tested to be sensitive to gentamicin (MIC = 0.5 mg/L). The apparent volume of distribution is 0.3 L/kg and the patient has a normal renal function as evidenced by her serum creatinine (Screat) of 0.9 mg/dL. Assuming the drug is well described by first-order kinetics and may be described by a one-compartment model, calculate the following: a. The expected Kel of gentamicin in this patient using the following gentamicin nomogram (Ducharme MP et al, Pharmacie clinique et Therapeuticque, 3rd ed) kel = 0.01 + 0.003 x ((140-Age) x 0.85)/Screat b.
