Shelbyville glass factory faces $230K in new safety fines AP SHELBYVILLE, Ind. -- A central Indiana glass factory hit last year by a record $453,000 fine after a worker was fatally crushed in machinery is facing another big workplace safety fine. The Shelbyville News reports federal OSHA officials issued a $230,000 fine Monday against Pilkington North America for an improperly installed laser safety system that should have locked down a machine which crushed and injured a worker in October. Pilkington now faces a total of $833,000 in compliance sanctions in the past year against its Shelbyville automotive glass plant. OSHA fined the company $150,000 last spring and a record $453,000 in August for safety violations not corrected after a worker was crushed to death in 2010.
