A hospital with four radiographers and several assistants requires a system that will help them to administer
patient records. The system only administers patient's information and no records are stored by the system. A
patient record consists of the patient's name, date-of-birth, gender, date-of-first-visit, date-of-last-visit, type of scan
done and patient diagnosis. Records of patients are grouped under next of kin. The next of kin has attributes such
as name, surname, address and phone number.
In the hospital each individual radiology staff member has a record of who works with a patient. Information about
the staff members is a kept to a minimum because the system mainly focuses on patient administration.
Information concerning each patient visit such as date, fees actually paid, and the type of scan done is stored on
the system. Each visit is for one patient, however a patient can have many visits in the system. More than one
radiographer may be involved in doing a scan during each visit.
The system keeps track of invoices. The invoice shows the type of scan done, amount paid and total due.
Information about the patient is also tracked by the receptionists and is entered into the system. Invoices are
printed monthly by the Account Officer and sent to the patients. Radiographers are responsible for capturing
information about the type of scan they perform and the diagnosis of the scan. 1.1 List the actors that participate in the above hospital system.
