A loose Acknowledgment or jurat certificate:

A.should be completed and attached to every document that a borrower signs.

B.should be completed and attached to a document when the notarial wording on the document does not comply with your state law or when the document in question does not have notarial wording printed on the document.

C.should be completed and attached to a document in addition to completeting the notarial wording on the actual document just for backup

D.should never be used because the notarial wording must always be included on the document for the document to be valid.
