A powerful explosion and subsequent chemical fire killed four employees, injured 32 people and destroyed T2 Laboratories, Inc. (T2), a chemical manufacturer in Jacksonville, Florida. (Watch this CSB video online) Assume that you did your summer internship at T2 laboratories prior to the accident. Your supervisor asked you prepare a detailed HAZOP analysis. Identify as many failure modes as possible. (Failure modes should include reactor, cooling line, feed and operating instructions). Perform HAZOP analysis using all identified study nodes.
