A social history will vary from place to place. There are certain topics that are always included but depending on the type of treatment an agency delivers emphasis on different areas will vary. On page 307 through 310 you will see a general example of a social history. Using this format present a social history on yourself or a fictional person. No social security numbers need included just use 000/00/0000. This should be at least two pages.
