According to the APA the deinstitutionalization movement of the 1960s which shut down large treatment facilities for the mentally ill coupled with the lack of community resources to treat them, resulted in some people going to prisons and jails instead. Mental illness among today's inmates is also pervasive, with 64 percent of jail inmates, 54 percent of state prisoners and 45 percent of federal prisoners reporting mental health or substance abuse or a co-occurring issue of both (National Research Council 2014).

You are responsible for a transitional treatment plan for a client with a serious mental illness and a history of substance use. What do you feel needs to be in that plan to help the client transition from prison into the community and not recidivate? Please explain why you selected those interventions?
