There is a machine at your workplace that does the final wrapping of the product. When it is not working, the safety procedures call for the plug of the machine to be disconnected from the mains power supply. On Monday a worker was hurt while cleaning the machine as somebody inadvertently switched the machine on. The supervisor reports that the power supply of the machine was not disconnected while it was being cleaned. This is the second time it has happened – luckily the first time nobody was hurt. You have been asked to investigate the matter and write a concise report (your company has no standard format for such reports) to the manager of the department. Hints for the report: Title of the report Introduction/Background: Purpose of the report. Identify the problem. Body of the report: Analysis of the situation, how it happened, who were involved, why safety procedures were not followed, cost for the company, eg, compensation for the injured party. Alternative solutions. How to avoid this in the future There is a machine at your workplace that does the final wrapping of the product.
