Advanced Directive

The purpose of this assignment is to familiarize you with the Texas DIRECTIVE TO PHYSICIANS AND FAMILY OR SURROGATES (also called an "Advanced Directive for Medical Care" or "Advanced Medical Directive", and in some states other than Texas, a "Living Will").

Part Two:

Fill in the blanks and make choices indicated in the document. Submitting a blank form is not sufficient and deductions will be made! Attach your completed form below for grading.
