After learning about Columbia's final mission, the air disaster at Tenerife, the Toyota Acceleration Pedal controversy, and the events on Mt. Everest in 1996, make report that compares and contrasts two of these events. You do not need to provide extensive summaries of each case. Instead, think about the following:

• Use course concepts to compare and contrast the two cases you are focusing on. E.g. Were their examples of confirmation bias, bounded awareness, escalation of commitment, etc. Were some concepts more relevant to one case than the other? In terms of course concepts what makes the events similar? What makes them different?
  • How might you compare them across levels of analysis?
    • The individual-level decision-making biases that contributed to the disasters
    • The team-level factors (or processes) that contributed to the disasters
    • The systems-level factors and types of complexity that contributed to the disasters
• What are some lessons/takeaways for you and others?
