Subsequently, the company faced many legal issues from several medical authorities in the U.S. In 2018, the company officially stopped operating. b) Therac-25 was a radiation therapy machine that was involved in several accidents where patients were given overdoses. After investigation, it turned out that the software was not reviewed properly, the system did not provide appropriate feedback to the user when there was something wrong, and the company did not test the machine after integrating hardware and software until it was assembled at the hospital. One of the contributing reasons to this crisis was because the operators of the machine did not believe the complaints at the beginning which was likely because of their overconfidence. c) A computer company is working on a flight reservation system for an airline company. The system is supposed to be used by different departments, to control all the details of the reservation process. A quality assurance software engineer is concerned that some of the functions of the system are not tested sufficiently. The software engineer reports to the managers but they try to push the engineer into signing the systems compliance with standards by stating that the company is at the risk of closing its door. Being pressured by the managers, the engineer signs the document. d) A company signs with a series of candy shops to automate their daily processes. The software development company forecasts a delivery date that cannot be met unless the staff works 24/7, or the development process is sped up by setting short the requirements definition phase. e) A development company is working on a Learning Management System for a university. The company rushes over the development phase to meet the deadline resulting in a poorly written code with no documentation. The product is delivered to the customer but with a low quality that does not satisfy the customer. f) Facebook was confounded by multiple people including Mark Zuckerberg, and Edwardo Saverin. Saverin was responsible for running Facebook's business part while Zuckerberg was working on the product. However, Savarin was feeling left out and froze Facebooks bank account. Zuckerberg decided to restrict Saverins decisions in the company by reducing his shares.
