Describe the religious belief and/or creed and explain why you are unable to be fully vaccinated against COVID-19 due to your religious belief/creed. Please note that under the Ontario Human Rights Code, only beliefs, convictions, and practices rooted in a creed or religion are protected. Your declaration should include sufficient and detailed information to demonstrate that the reason for your request to be exempted from the application of the COVID-19 Mandatory Vaccination Policy has a nexus with sincerely held personal beliefs that are experientially religious in nature or prohibited by your religion/creed. Secular, socially based, or conscientiously held beliefs are not protected under the Code and will not be approved as an exemption to the university's mandatory COVID-19 vaccination requirement.
