The client (an insurance company) has hired your Pl firm to investigate a potential case of insurance fraud for a personal injury. You are to conduct surveillance on the subject for 3 consecutive days for at least three (3) hours per day and document/report your observations for those days. You are to make up the name of the Private Investigation company you work for. You are to make up the name of the Insurance company which has hired your firm. You are to make up the subject''s name, gender, DOB, and any relevant contact information for the subject. Your report should include relevant photographs and copies of important surveillance notes (from a memo book or similar source). The subject description should match the subject within the photograph(s) included in the report. Scenario The subject was reportedly in an automobile accident on 2/5/2021. The subject claims they are unable to work because of the accident. The subject claims that as a result of the accident that they are: - Unable to walk more than 20ft without taking a break or using a support. - Unable to lift objects more than 10Ibs. - Unable to drive a vehicle and must take another form of transport. Your surveillance dates should be 2/11/2021, 2/12/2021, and 2/13/2021. You primary point of surveillance is the subject's home. During at least one day of surveillance, the subject should leave their home and depart in a vehicle and you as the Pl should follow them in your vehicle. You can either find that the subject HAS or HAS NOT committed insurance fraud. The investigation summary should include a paragraph outlining the basic facts 2 points of the case (including the claims by the subject). The investigation summary and the detailed report must have all major events in 1 point numbered paragraph form. All last names within the report must be capitalized. 1 point The detailed report should have an accurate description of the subject (eg. what 4 points are they wearing, details should match the subject information section). The report must include all relevant times related to the incidents and dates and 4 points times must be in a consistent format throughout. The detailed report must be written in chronological order. 2 points The detailed must include a date and signature by the Investigator on each page. 2 points Must include photographic evidence of the incident (at least 1 image per date of 6 points surveillance). (2 per day) Must include a copy of notes from your memo book. 4 points Your detailed report should include events which tie back to all claims the subject 6 points has made. These should support the case either for or against the subject. (2 per claim) Detailed report should include a paragraph for each day of surveillance indicating 3 points the time the PI arrived on site at the subjects home, as well as what time they (1 per day) ended surveillance that day. Detailed Report should include an entry related to the subject departing in a 3 points vehicle and the mobile surveillance that followed (documenting the type of vehicle, direction of travel, etc.) The entire report should not include any opinions. Only facts. 2 points TOTAL 50 POINTS
