At my facility, we us PDSA on a unit level as a form of QI that employees and management can identify the issue and come up with quick possible solutions to trial. For example, in our surgical unit, we had an issue with PACU being too full too often due to the high volume of quick total joint patients which caused other patients to have to remain in the OR suites to recover, thus charging more money for the patient, decreasing the amount of cases that can be completed, and delaying patients who are scheduled for surgery that day. The plan was to transfer the joint patients to a recovery room set up in the rehab unit which would be their next stop anyway. A PACU nurse would be stationed there and help recover them until rehab took over. The plan was set and it was time to follow through and it did not work out well. Based on patient and staff feedback, changes were made and trials were conducted again. This solution works for the meantime; however this was a band aid, and the real solution was to expand PACU and staffing, which obviously takes months to years of planning and executing
