Barb is a 76-year-old woman who has just been moved from hospital to a residential rehabilitation facility in Queensland. Barb was in a terrible car accident which resulted in the death of her husband of 20 years Alphonse (Alf). Barb sustained a traumatic brain injury (TBI) during the crash, as well as other injuries. Due to her TBI, Barb has aphasia and issues with cognition and executive functioning. It is hoped that with time and rehabilitation Barb may make some gains in her functioning. At present she has been assessed by a team of specialists as lacking the capacity for decision-making - for personal care and well-being and financial matters. Barb does not have a power of attorney for either personal health and well-being (health matters) or finances. She also does not have an advance health directive. She is survived by her daughter from a previous marriage, Ebony, and Ebony's two children - Levi (20) and Logan (15). The family are close, and Ebony, Levi and Logan saw Barb and Alf almost every day and had since the children were born. Barb's admission was processed by Mindy Chen, a nurse practitioner, in the presence of Ebony and Levi. Mindy noted that although Barb had received multiple COVID-19 vaccinations, she had not received a booster for more than 6 months and had not had a recorded COVID-19 infection during that time. She had received all other childhood vaccines, all available adult voluntary vaccines, including annual flu vaccines, and boosters of these where appropriate/required with no more than negligible side effects, such as sore arms. Mindy noted that because of her age and her injuries Barb was in a higher risk category and recommended the COVID-19 booster be administered. Before Mindy finished her sentence, Ebony jumped in saying: "there is no such thing as COVID. It's just big pharma inventing something so they can profit, and government supporting them to take away our freedoms. They put trackers in the vaccine and use the cells of dead babies to make the vaccines. They are using the money to traffic children". Behind his mum Levi rolled his eyes. Ebony went on to say "I absolutely don't agree, mum can never have the vaccine, I refuse! You should do your research - you are a disgrace as a health professional". Earlier in the admission process Mindy asked about Barb's activities and values. Ebony and Levi had stated that prior to the accident Barb and Alf volunteered two mornings a week at the local hospice and had for years. They continued this during COVID-19 taking great precautions to keep patients, families, and staff safe. When Ebony stepped out briefly to take a phone call, Levi told Mindy that keeping vaccinations updated and wearing masks everywhere was really important to Alf and Barb because they really cared for people and didn't want to be responsible for anyone getting seriously ill or dying. Barb also volunteered for a service that organised volunteers to go into the homes of persons with dementia to keep them company to give their carers a respite break. She took the same precautions in that context. Levi and Ebony emphasised how caring Barb and Alf were about others.
