Scenario:
Janine is a7-year-old female diagnosed with autism spectrum disorder. She is currently homeschooled by her mother (due to medical reasons) and receives approximately35 hours per week of ABA services funded through her insurance. Throughout her day, she wears ahelmet due to self-injurious behavior in the form of her fists directed towards her head. She communicates through vocal approximations that are very difficult to understand, even to a familiar listener, as well asTouchChat (an Augmentative and Alternative Communication application on her iPad). She canindependently navigate through the pages and communicate with full sentences when handed her device and asked "What do you want?". However, she doesnot spontaneously initiate requests.
Lately, the staff have noticed an increase in frequency and intensity oftantrum episodes. Tantrum episodes often will include bolting from the designated area, jumping, yelling, and self-injurious behavior. ABC data indicate that the tantrum episodes are lasting approximately45-60 minutes, are preceded by demands to engage in leisure activities or work activities85% of the time, and are consequated by a lessening of the original demand, delay of the original demand, or an alternate demand being presented. Preference assessment indicates that she enjoys time in herrocking chair, wandering around the house listening tomusic, and having mom rubessential oils or lotion on her arm.
