BATTERY REPORT NARRATIVE MUST-HAVES

Include in this narrative:

• Date and time of your call? What were you called to?
• Where did this happen (business name and address)?
• What happened from your point of view (what did you see)?
• What happened according to each person there?
• Statement of non-consent from the victim
• Was there bodily harm (that includes physical pain)?
• Did you issue the Crime Victim Information Sheet (list it in the narrative)?
• Did you collect any evidence (pictures of injuries)?
• Did you issue any charges? If so, to whom and what did you charge them with?
