Bob has been hospitalised several times over the years due to major depression, schizophrenia, and periods of suicidal ideation.

During Bob's most recent involuntary admission to an inpatient unit he felt disrespected; that he didn't get to have much input into the treatment he received.

Bob has been engaged with your service for a few months as part of Bobs most recent clinical mental health care plan. Bob wants to make an advance directive for future hospital admissions as he is worried that no one knows his preferences for care and treatment.
