Business Scenario A health provider submits a pre$-$authorization request to a health insurance company for a patient$$s Magnetic Resonance Imaging $($MRI$)$ scan at an Imaging Center. At the health insurance company, the Claims Service department reviews the request documentation for completeness and files the request. If the request documentation is incomplete, the Claims Service department rejects the pre$-$authorization request. Assuming the documentation is complete, the Claims Handling department then verifies whether the patient$$s insurance policy includes coverage for the MRI scan. If the policy lacks coverage for the MRI scan, the Claims Handling department rejects the pre$-$authorization request. If the policy includes coverage for the MRI scan, the Claims Handling department evaluates the pre$-$authorization request. If the request is approved, the following activities occur at the same time: $(1)$ both the health provider and Imaging Center are notified that the pre$-$authorization request for the MRI scan is approved and $(2)$ the Claims Handling department authorizes payment for the MRI scan. If the preauthorization request is denied, only the health provider is notified that the request is denied.
