CAREER ACTION PLAN TITLE PAGE This should be standard college format A sample is provided on Moodle TABLE OF CONTENTS CAREER OBJECTIVE/MISSION STATEMENT: This is a clear, concise statement that accurately articulates your ultimate career objective. By defining and writing this it will help you stay on track until you successfully accomplish this mission. Create your Mission Statement for your co-op work term. 4. GOALS & OBJECTIVES Writing "S.M.A.R.T." goals will serve as the major steps that will lead you to successfully accomplish your ultimate career objective or mission statement. Your goals need to be, Specific - Measureable - Attainable - Realistic - Time Bound. In addition, for each goal, writing "S.M.A.R.T." objectives will serve as the specific, practical steps that, when taken, will lead you to successfully accomplish the goals they support. Write a minimum of 4 "S.M.A.R.T." goals and for each goal, identify a minimum of 3 "S.M.A.R.T." steps. STRATEGIC PLANNING - VISUAL TIMELINE/PERT CHART Establish timelines; assign specific dates and responsibilities to each item on the chart/timeline leading you to achieving your ultimate career goal. IMPLEMENTATION/FOLLOW UP Present this completed document to someone who will be an accountability partner with you help you evaluate and attain your goals. On your finished document you will determine what this accountability structure will look like and establish a timeline for the accountability follow ups (for example: Once a week, once a month, etc.) Have your accountability partner sign and provide their student number at the bottom of that page indicating that he/she agrees to partner with you in helping you attain your future goals
