Case 3: An elderly patient is brought by ambulance to the emergency department in cardiac arrest suffered at home. The ED physician provides and documents care services to the patient for a total duration of 2 hours before the patient is handed off to an attending physician who admits him to the Cardiac Care Unit (CCU).
