Case # 4 Joseph Joseph, a 19-year-old African American college freshman. Yesterday he spent the afternoon drinking beer and taking shots of vodka with his fraternity brothers. After 6 glasses of beer and 5 shots of vodka, he becomes argumentative with one of his larger companions and suggests that they step outside and fight. Normally a quiet, unaggressive person, Joseph now speaks in a loud voice and challenges the larger man to fight with him, apparently for no good reason. His speech is barely understandable as his speech is slurred. When the fight does not develop, he becomes morose and spends long periods looking into his beer glass. He seems about to cry. After more beers, he begins telling long, indiscreet stories about former girlfriends. His attention quickly drifts when others talk. He tips over several beer glass, which he finds humorous, laughing loudly until the bartender gives him a warning look. He starts to get up and say something to the bartender, but trips (over nothing) and falls to the floor. His friends help him to the car as he is unable to walk on his own. Back at the fraternity house, he falls into a deep sleep and when his friends check on him they are unable to wake him and Joseph begans to vomit uncontrollable. His friends take him to the nearest emergency room and as the clinician on call you are also called to conduct an assessment once Joseph is coherent. This is the first time that Joseph has ever become drunk. He grew up with parents who were very strict about alcohol and drug use, and he hung out with high school friends who rarely drank.
