Case Analysis: M and M Clinic M and M Clinic is one of thirty clinics in a community with a population of 300,000. The average income for this community is $38,000. Many residents of the community are employed in several large distribution centers. the local university, or in one of the many retail and service industries. The clinics offer general and special medical services. Market share for the group ranges from 2-10%. Market share for M & M Clinic is 2%. The number of physicians in each clinic ranges from 3 to 10. Staff at M and M number about 15. The range of staff in each of the other clinics is 10-30. Seventy percent of the physicians at M and M are white men. Others are African-American, Hispanic, or Asian. Some physicians are women. The clinical employees are composed of 50% African-American, 25% Hispanic, and 25% Caucasian. The clinical staff are 80% women. Employees at M and M have complained their paychecks have not been accurate. Management has confirmed the problem, blaming it on an outdated computer system, and corrected the problem, but they reoccur. Patients have also confirmed their billings have been inaccurate, but corrections are eventually made. The computer system was again blamed. Similarly physicians have found the computer system to be unreliable in producing medical records timely and not always reliably accurate. Billings for reimbursements from government and private insurance companies have resulted in staff and physicians spending additional time making corrections in the computer system to get all the revenue the clinic is entitled Medical errors at M & M run 15 per month, above average for the group. The range runs 2 to 15 per month. Employee turnover for M and M is 15%. Average employee turnover for the others is 5%. The federal government through the Center for Medicaid and Medicare has been wanting all facilities to coordinate services and move to electronic records. CMS has offered to fund grants to clinics who demonstrate working together or merging to update their records systems and providing better patient coordinated services. Three other clinics have been receiving negative press over a malpractice scandal recently. Two other clinics have had someone embezzle money. All these clinics and a few others have asked whether M and M would like to merge or coordinate services more with them. While M and M has been approached about several clinics merging, another group of 5 clinics are rumored to be discussing merging as one clinic. M and M also has a former disgruntled employee who has been hired by the state's office of medical investigations. Other employees still with M and M have told the physicians this employee vowed to get even with M and M by revealing damaging information to the state investigations office.
