Cause-and-effect Diagramming (Assignment package): Background: The telephone survey and Pareto charting have provided the VivosysQ Project team with a good idea for the chief reasons why customers are unhappy with the technical assistance they are being provided. However, the survey does not tell the team why these problems are occurring. They decide to use cause-and-effect diagramming to get at the root causes. Question: Construct a cause-and-effect for each reason for dissatisfaction that you decided to correct from Part 2. Use the major service causes of Policies, Procedures, Plant, and People as your starting point. Show at least three minor cause per effect. Show one root cause for each minor cause
