T.H. is a 15-year-old male adolescent who has had attentional issues, impulsivity, and hyperactivity since childhood. These symptoms have affected his academic achievement and relationships with classmates and family. The symptoms appear in family and educational contexts.

How are their symptoms impacting their functioning in life?

T.H. has trouble focusing in class, gets distracted when doing assignments, and is restless and fidgety. He differentiates between impulsive activities like spontaneously responding in class or interrupting talks. An overworked mind causes problems falling and staying asleep. T.H. has sleep problems, including trouble falling and staying asleep. Due to his declining academic performance, he is angry and overwhelmed. The symptoms have been there for years, but greater academic expectations have worsened.

Assessment:

Mental Status Examination: T.H.'s general appearance was unfocused during the session. His euthymic attitude fits the situation. Cognitive processes proceed logically. He lacks suicidal or homicidal thoughts or intentions. Focusing is arduous; therefore, the discourse often wanders. T.H. was presented as polite, curious, connected with the provider, and wise during the session. T.H. had a consistent, balanced mood and proper emotional expression, which eliminated the possibility of significant depression. The assessment showed that he thought linearly and logically. Cooperative and inattentive throughout the session, which was also evident, supporting the main issue. VS:98.0, 19RR,118/76BP, 71HR, 0/10 pain

Medical History:T.H. was first diagnosed with ADHD when he was 14 years old. He started treatment through cognitive behavioral therapy and was prescribed Strattera 40mg daily with no side effects. This non-stimulant boosts his norepinephrine levels in the brain, which helps boost motivation, attention, and awareness; however, he has not been consistent in taking his medication due to forgetfulness.

Objective:

Diagnostic results: Evaluations include the Vanderbilt ADHD Diagnostic Parent Rating Scale, in which the patient scored two items of the test, a 4 and 5, showing there must be some impairments. According to the scores, the T.H. is likely to have ADHD, specifically inattentive presentation.
