Chapter 5.6 Bow-Tie Diagram: A loss incident occurred during construction of the Tappan Zee Bridge in New York around noon on July 19, 2016. A crawler crane was carrying out installation of steel piles in the Hudson River for the new replacement Tappan Zee Bridge. The piles were installed using a vibratory hammer. The crane boom collapsed during the pile driving process and fell over the existing and active Tappan Zee Bridge, stopping active traffic on both northbound and southbound lanes. As a result, traffic through the bridge was shut down and diverted after the incident. There were a few minor injuries reported, with no major injuries or fatalities observed
