Thiscanbeevidenced bythelearnercompletingtheprovidedquestions basedontheirinteractions,aswellasprovidingcopiesofclientcasemanagement plans (all names and personal details to be removed prior to submission - consent from organization supervisor/manager and client must be obtained).

1. Consistentlydisplayedtheabilitytomeettherequirementsoutlined intheSupervisorObservationsection ofthisunit
   • Thismustbeevidenced acrossthe3casemanagementmeetings,wherethelearneristouseindividualizedcasemanagementplansastheirbasisfor providing support to their clients

Foreachoftheseclientinteractions,thesupervisorwillneedtocompletetheobservationchecklist,providethedateofobservationandadditionalnotesto support their indicated outcome (examples of relevant notes can be found in the 'Instruction to assessor'section of this document).

Duringtheirtimeonplacement,thelearnerwillneedaccesstothefollowingorganizationaldocumentsandresources:

• Suitablefacilities
• Equipmentandresources,including individualizedcaseplans
• Scenariosthatreflectstandard operatingconditions
• Localserviceagenciesor organizations
