Summary of Reflections The worker contemplated the incident and thought about waysthesituationcouldhavebeenaddressedmoreeffectively. The worker believes that she could have been more assertive with the client, if she had established boundaries with him earlier on. The employee also believes that the incident could have been avoided if the client had been provided with more structure. Upon reflection, the worker feels that she could have called the police sooner.

Learnings:The employee gained knowledge and understanding that it is essential to ensure the client's safety, thatclear boundaries are established with clients from the very beginning of their time together and to contact the authorities at an earlier stage if the situation worsens. The client's frustration with the staff assigned to the house should be addressed immediately to prevent another incident from occurring. It's possible that the incident could have been avoided if the client had been provided with more direction and structure. The worker has established the need for further training and support

Actions Taken so far:

The worker called the on-call supervisor and the police.

The client was arrested and taken to the police station.

Informal debrief by On-call supervisor immediately after the incident.

Formal debriefing today with the worker.

Actions needed:Further support and professional counselling needed for Jessica Smith. Crisis-Prevention Training is provided to every worker, and refresher training is to be attended annually. Property to be assessed for repairs and insurance before the client is released from police custody. Employee rosters are to be re-evaluated and a skills audit of staff is to be conducted. Needs analysis to be conducted for critical incidents occurring at the house. Workplace policies and procedures to be updated. The worker is to attend referral to E.A.P. Follow-ups with the worker are required on a regular basis.
