Choose a problem that you have experienced in your personal life or at work. Ensure the problem is simple/small in scope so that you will be able to use it as a basis for a simple PDSA cycle in your next unit assignment. Feel free to reach out to your faculty member if you need advice on the scope of the problem you would like to use. Describe the problem including at minimum these details:

  1. What is the problem?
  2. Who is involved?
  3. Where does the problem take place?
  4. When does the problem occur?
  5. Craft a problem statement, using the information you learned in this unit about developing effective problem statements.
  6. Draft an aim statement that meets the SMART criteria to identify what you would like to achieve. !Tip: remember, the aim doesn't seek to identify the source of the problem yeti Summarize why/how you feel your AIM statement meets the SMART criteria.
  7. Using the fishbone diagram template available on the Gateway in Unit 3 Tools, brainstorm possible root causes that are contributing to the problem Document the root causes on the fishbone diagram. You may use the 5 Whys to help you drill down to actual root causes, but this is not mandatory.
  8. Identify which root cause you will tackle in your next unit assignment and explain why you chose that one. Because the problem is simple, you don't likely need to prioritize using a Pareto Chart, but feel free to do so if you wish and share the results with your assignment submission. Creating a Pareto Chart is not mandatory or marked.
  9. Share a very brief reflection of how you feel this process went. What was easy? What was hard? What did you learn for next time?
