Written description of the process: Describe the steps you took to identify and select the issue. This could involve initial research, speaking with stakeholders, surveying a group, analyzing data, etc. Outline the overall process you followed.

List of brainstormed ideas with stakeholders: Provide any lists, charts, or images showing the ideas that were brainstormed when exploring potential issues to address. This helps showcase the range of ideas considered.

Copy of survey tool: If you conducted a survey, include a copy of the blank survey you distributed to help identify the issue.

Final survey results: Summarize or show the final results of any surveys conducted. Charts, graphs, or data visualizations can help present this.

List of brainstormed solutions with stakeholders: Similar to the brainstormed ideas, provide visuals showing the potential solutions brainstormed for addressing the selected issue.

Description of the issue and why chosen: Clearly describe the issue you ultimately decided to focus on and address through this project. Explain the key reasons why this particular issue was selected as important and impactful.

SMART goal: Draft a SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goal statement for what you hope to accomplish by addressing this issue.

Barriers/challenges and possible solutions: Anticipate any potential barriers, challenges or obstacles you may face in executing this project. List them out, along with possible solutions for overcoming or mitigating those challenges.

Project/event description: Describe the actual project, event, or activities you plan to take on in order to address the identified issue. Outline the key components.

Timeline: Develop a projected timeline for completing this project, hitting key milestones, etc.

Task list and responsibilities: list of the major tasks that need to be completed, with details on who will be responsible for each task.

Resources: List out any people, groups, organizations, or other resources that can assist with this project and how they may be able to contribute.
