Siobhan:
Siobhan is a 30-year-old Irish American female. She was admitted to a hospital after a suspected suicide attempt. Siobhan had consumed too much alcohol and was being treated for alcohol poisoning. Siobhan's parents stated that she has suffered from mood swings since she was an adolescent, and they thought she often used alcohol as a way to cope with her fluctuating moods. They stated that Siobhan used alcohol frequently and to excess. Siobhan had never been treated for her condition, but her parents stated that she often alternated between deep depressive states and states where she was out of control, staying up for days at a time, engaging in risky behavior, and having grandiose thoughts. At times, Siobhan scared her parents because they also thought Siobhan heard voices that told her to do things. Her parents were always very worried about her, but they could never get Siobhan to agree to treatment.
Rodolfo:
Rodolfo is a 25-year-old Italian citizen who is in the United States on vacation. He speaks very little English. During one night while he was out celebrating a friend's birthday, Rodolfo drove his rental car while under the influence of alcohol and hit and killed a pedestrian. Rodolfo was arrested and convicted of involuntary manslaughter. The judge ordered a substance use assessment to determine sentencing for Rodolfo. What the judge wants to know is whether drinking and driving was a one-time occasion for Rodolfo, or whether it was the result of deep-seated substance use disorder behaviors.
Steve:
Steve is a 16-year-old Caucasian who is native to the United States. Steve began drinking in middle school, and his drinking has gotten out of control. Steve has had problems with continuing his education due to his drinking behaviors and was formally kicked out of his last school for being drunk on campus. Steve currently has multiple educational deficits and only reads at the seventh-grade level. Presently, Steve is in an alcohol dependence treatment center for his chronic use of alcohol. Steve has been unresponsive to outpatient treatment and is largely in denial about his drinking. He deeply resents his parents for interfering in his life and for putting him into treatment. Steve is uncommunicative and unwilling to discuss his use pattern, but did consent to taking an assessment.
