Client Information and Presenting Issue:Sarah. M (SM) is a 28-year-old Black African American Divorced female and a mother of two children under the age of 12 years. She was referred to me as her therapy/counseling counselor. She is on a court-mandated treatment for substance use addiction and mental health disorder in the River Community after her release from jail. Sarah has been dealing with concurrent challenges related to substance abuse, mental issues, and homelessness. She is now in the care of the agency and has been admitted to our structured inpatient program for a minimum of 12 months, which includes medical treatment, individual and group therapy, and learning sessions. She is highly motivated to engage and has actively participated in all group learning, Alcoholics Anonymous meetings (AA), medication management, and follow-up appointments. She has indicated that she is committed to completing treatment to regain custody of her two children because they have been taken away by the Department of Children and Family Services (DCFS) during her addiction and jail time. Relevant Information:In my field education experience as an intern Counselor at the Social Model Recovery System Agency, River Community, a co-occurring disorder treatment facility, conducive to addressing both substance use and mental health issues. I had the opportunity to be the counselor of Sarah. M and our meeting were arranged by the program director of which Sarah M. was duly notified. The agreed-upon date is every Saturday at 1 pm. SM:(Nods) Ms. Stella, it's just... I feel overwhelmed, you know? Dealing with a lot in my life, anxiety, being diagnosed with bipolar, though I do not agree with that, and then trying to stay clean and not allow people to control how I react around here. It's a lot.
S: I can imagine it feels like a lot to handle. What specific situations trigger your anxiety, and how can we work together to manage those?
SM: I am determined this time to be the best, maintaining my boundaries. I will give anyone the power to control how I act and also will respect others by being self-aware.
