Submit all three Coding Clinic topics in one document/submission 2. Your submission should be formatted as noted below. Put any direct quotes in quotation marks. You do not have to identify the reference since the reference is only Coding Clinic 3. Do not copy and paste any Coding Clinic or write verbatim, but summarize your findings, i.e., make believe you are explaining this to someone who does not know anything about coding. No credit for plagiarized work. 4. Locate three separate Coding Clinic topics/subjects of your choice. These can be anything that interest you: procedures, coding processes, guidelines and rules, frequently asked questions (by a specific topic), etc. 5. Your submission must follow the required format to receive full credit. Format each (all three) Coding Clinic topics/subject in the same manner. Clearly number your paragraphs, 1-4, for each case points deducted for those not in paragraph form and/or numbered. Be sure all numbers for each submission state 1-4. If missing characters, points will be deducted. Format All must contain the following Name: Coding Clinic Assignment Code Number and Description: Coding Clinic Reference: Provide the Month, Year, Page Number, and Category, if applicable FINDINGS (create and number a separate paragraph for each below you must address each question for full credit): Summarize the information given such as any questions/answers provided (do not copy/paste into your answer sheet). If there is an example, provide at least one. If none, write N/A. Identify any associated ICD-9 or ICD-10 codes connected to this noted in the content. If none, write N/A. It is preferable that you select Coding Clinics from ICD-10-CM/PCS; however, since ICD-10-CM pulls coding guidelines from ICD-9-CM, you may use those as well.
