-You are employed as a staff accountant aT XXX Tax Preparation, Inc.; .; TIN 43-9892911 -You have wages of (Digits 4, 3, 5, 9, 1) plus a taxable bonus of $35,000. -On your W-2, you need to fill in the EIN and address for the business as well as yourself. Do not worry about a control number. You need complete lines 1, 2, 3, 4, 5, 6, 15, 16, 17, 18,19, 20. -Line 1 is Taxable Wages for the year. -Line 2 is Federal withholding on your wages. Please withhold 11% of your wages as taxes prepaid to the IRS through the pay as you go withholding system. -Line 3 is Social Security Wages. Since your wages are below the $162,000 threshold for 2023, this amount will be the same as your Taxable Wages for the year. If in a future tax return, your wages are in excess of $162,000 then your Social Security Wages are capped at $162,000. -Line 4 is Social Security Tax withheld. This is 6.2% of the social security wages. If in a future return, your wages are in excess of $162,000 then only the first $162,000 is subject to this tax. -Line 5 is Medicare Wages. Since there is NO upper limit on the Medicare tax and you do not have taxable tips, this amount should be the same as the Taxable Wages. -Line 6 is Medicare Tax Withheld. This is 1.45% of the Medicare Wages on Line 5. -Line 15 is the state in which you have earned taxable income (PA) and the Employers State ID number. This number is typically different than the Federal TIN, but for these tax returns, please use the Federal TIN. Line 16 is state wages and tips. Since you are only working in one state, all of your federal wages are included in this state. Line 17 is the state taxes withheld through the pay as you go withholding system. You are all residents of Pennsylvania and must withhold 3.07% of your wages, Line 18 is local wages earned. You will need to allocate the wages across all locations in each state that you worked and earned money. For this tax return you will only work in one location (Mt. Lebanon). All of your state wages are allocated to this single location. Line 19 is local income tax withheld through the pay as you go withholding system. The tax rate in Mt. Lebanon is 1.3% of all wages. Line 20 is the locality name Mt. Lebanon You earned interest from PNC Bank (TIN-23-1123211; Mailing Address: 835 Fifth Ave., Pittsburgh, PA 15218; Phone Number 1-800-226-5707) in the amount of (Digits 10, 2, 4, 5) plus $1,750.
