- Using data and information sources that you have available, identify a specific issue or problem that exists at a school site, district office of education, or the organization where you currently work (Data and information could be in the form of student learning issues, specific site/organization issues that may be prohibiting the organization from moving forward, PLC Professional Learning Community/grade level learning data, school or site/organizational level surveys, parent/worker information, your own observations, and similar items.)
- Establish a minimum of 2-3 goals/objectives that you would like to pursue during this year to address the issue/problem (What do you want to accomplish?)
- Identify the activities (minimum of 2-3) that you plan to pursue in order to address the issue/problem (How or what specifically will you do to help you accomplish your goals?)
- Identify your timeline as to when you want to accomplish your selected activities (By when do you want to accomplish each of your goals?)
- Identify the resources (such as monetary and people support) needed to help you accomplish the activities you plan to pursue (Identify the resources you will need to help you accomplish your goals)
- Identify how you will evaluate and assess progress made throughout the school year (Formative) as well as at the end of the school year (Summative).
- You may use References in addition to our class textbooks, as long as they are relevant and from year 2000 or newer.
