1. Client basic demographic information (i.e, 32 y.o. AA f. with 2 children ages 8 and 10). 1 minute.
2. Presenting problem(s): Client presented with a range of clinical symptoms inclusive of substance use (primarily alcohol and marijuana), depressive mood, anxiety, psychosocial problems including financial and legal. 1 minute.
3. BRIEF narrative. Do not read the story and retell the details of your sessions with the client. Briefly describe your objective experience with the client. 4 minutes.
4. Mental Status exam results. 3 minutes
5. Diagnoses. Include codes and titles. 2 minutes
6. Evidence based treatment method you will use or are using. Do not read the treatment plan goals and objectives. 2 minutes.
7. 2 consultation questions for the group 2 minutes.
8. Discussion with group. 5 minutes.
