• Make sure to use following types of questions:-30points
• Text,
• Multiple Choice,
• Check boxes,
• Choose from list,
• Paragraph Text,
• Scale
• Grid.
• Other types are optional
• Customize confirmation message and require all questions to be answered -10 points
• Make your form public (when you test you link from other computers and browsers, it should work, make sure to uncheck Login Required under settings) -10 points
• Add at least 1 image and 1 video to your questions -30 points
• Relevancy of the topic (should be about school setting and not off-topic) -10 points
