Mountain View Community Hospital You need to design an ERD based on the following user requirements (90 points) The hospital has a number of Care Centers (e.g., maternity, emergency care). Each care center has a care center ID and care center name A Patient is admitted to the hospital by exactly one physician. For each patient we need to store the medical record number (MRN), name, and date of birth. Each physician has a physician ID and name. A physician may admit any number of patients and may not admit any patients If a patient is referred by a physician, we need to store this as well. A patient may be referred to the hospital by exactly one physician. A physician may refer any number of patients or not refer any patients Some of the patients (inpatients) are assigned a bed. Each bed has a bed number, a room number, and a care center ID. Outpatients are not assigned a bed Patients are diagnosed by physicians. Each diagnosis has ID/code and diagnosis name. Physicians diagnose any number of conditions affecting a patient, and a diagnosis may apply to many patients. A patient can be diagnosed by more than one physician. The hospital records the following information date and time of diagnosis, diagnosis ID/code, the name of a patient, and the name of a physician Patients are ordered treatments by physicians. Each treatment has a treatment ID/code and treatment name. Physicians may order and perform any number of treatments for a patient or may not perform any treatment. A particular treatment may be performed on any number of patients, and a patient may have treatments ordered and performed by any number of physicians. For each ordered or performed treatment, the hospital records the physician ordering the treatment, treatment date, time and results Employees of the hospital have employee number and name. Each hospital employee is assigned to work in one or more care centers. Each care center has at least one employee and may have any number of employees. The hospital records the number of hours per week that a given employee works in a particular care center Each care center has exactly one employee who is a designated nurse-in-charge for that care center. An employee can be a nurse-in-charge of a care center only if he/she is assigned to it. Prior to a patient being seen by a physician, a nurse typically obtains and records relevant information about the patient: weight, blood pressure, pulse, and temperature. The nurse also records the date and time of taking these vital signs. And finally, the nurse records the reasons for the visit and symptoms. Different medical and non-medical items are available and can be consumed by patients. Each item has an item number, description, and unit price. A patient may consume any number of items. A given item may be consumed by many patients or may not be consumed. For each consumed item, the hospital records the date, time, quantity, and the total cost. . . . . . . . .
