1. Create three (3) profiles of clients with complex needs:

• Describe each client, including their age, culture, gender, interests, living situation, and employment situation.
• Describe what reason they have come to you as a client for (e.g., alcohol or other drug dependency issues, family violence, unemployment, mental health issues).
• Describe why their needs are complex (e.g. cultural, religious, disabilities, linguistic issues)
• Describe what barriers may exist to providing them with services (e.g., mistrust, communication barriers)
• Plan which model of case management you will provide, including the services, resources and support.
• Include information about how you will assess each client (e.g. speak to them, read reports) to determine their needs. Explain how you will answer any questions or concerns the clients have.
• 2.Create 3 x plans (i.e., one plan per client) which coordinate multiple services and supports for their complex needs
• Include information about how to plan goals, sub-goals and timeframes.
• Plan around any issues faced by the clients or their carers. If necessary, begin a referral process to access other services.
• Be aware of your duty of care, and follow mandatory reporting requirements if necessary, such as if you suspect child abuse, domestic violence or self-harm.
• Once you have planned the support, provide information about the multiple agencies that you should contact to coordinate it.
• Explain how you will provide relevant information to them and work to overcome any barriers to the client achieving their goals (e.g. mental health, addiction, welfare dependency, mistrust).
• Explain how you will work with the client and services to prioritise their needs and adjust the plan where necessary.
