Fulton Schools of ASU Engineering Arizona State University DATE: (see schedule) SUBJECT: LAB ASSIGNMENT #6 - SITE SPECIFIC PROJECT SAFETY PLAN This task will require you to prepare a site-specific safety plan for the Phoenix Fire Station #59. This assignment will be a graded exercise for this class and may be used in the future as an assessment tool for future accreditation visits. Objective. The accreditation program for the Del E Webb School of Construction requires that every student receiving a BS degree needs to demonstrate the ability "Create a construction project safety plan" and "Formulate or design a system, process, procedure, or program to meet the desired needs." This lab assignment supports those Student Learning Outcomes. Process. For this assignment you will develop a safety plan which will be utilized as part of every subcontractor site orientation. The safety plan must contain information on PPE, trench hazard information, fall information, "struck by" dangers, crane safety, first aid and basic site dangers. Your site specific plan will be based on the City of Phoenix Fire Station #59 project. You will also be required to use 29 CFR 1926 OSHA regulations which are available at this link: https://www.osha.gov/laws-regs/regulations/standardnumber/1926 Safety Plan Content. The information listed below must be addressed in each plan and in the order listed. You may, and should, include other information; however, the entire plan must comply with the overall page limit. An example of a complete Site Safety Plan is provided on Canvas as an additional resource in completing this assignment, although your plans should only be 4-6 pages long. a. Emergency Contact list with names, positions, and phone numbers (10 points) b. Complete list of PPE to be used on site (10 points) c. Trench Safety - Evacuation / Use of Shoring / Ingress-Egress (10 points) d. Fall Danger Identification and Protection - Scaffolds / Lifts / PFAS (10 points) e. Struck- by Dangers - Crane Safety (10 points) f. Electrical - Power Cords and Temporary Power Requirements (10 points) g. Basic First Aid - Include information on nearest hospitals and urgent care (10 points) h. Basic Site Safety - Weather, Spiders, Muster Area (10 points) i. Provide a site orientation map to be used in briefing project visitors (15 points) j. Create a sign-off form which demonstrates understanding of site safety (5 points) Report Format. The safety plan will be written in the format of an internal company memorandum from you as the Project Manager to me as the Project Executive and will be submitted via Canvas using the assignment link provided. The report should include graphs, photographs, maps, and forms as necessary to communicate the aspects of your construction project safety plan. The needs of the report you are formulating is to inform and instruct all personnel that will be on site during the construction process. You are to format your safety plan in the same order as the Safety Plan Content section noted above.
