Profession 2 is a public health researcher. Profession 3 is a nurse case manager and Profession 4 is a receptionist at the eye clinic.

Problem Statement:

In September of 2022 Pharmalilly Chicago vaccine clinics have had reports of adverse reactions from one vaccine clinic. The clinic received 52 reports from patients within 24 hours of receiving the vaccines. Reports from patients include a range in symptoms from dizziness to nausea, which are side effects not documented at our other vaccine clinic locations. Patient safety is a priority at Pharmalilly and we are dedicated to the monitoring, evaluation, and reporting of all potential side effects and adverse reactions. All 3 Chicago clinics have been operational since January of 2022 all with reports of patient side effects that were present in trials. Reports of nausea, dizziness, and abdominal pain came from site location 2 from patients who were vaccinated between September 15^th and 21st , the vaccine clinic has been temporarily closed. Pharmalilly has been notified that 15 of the 52 patients reporting adverse reactions have been hospitalized citing nausea and abdominal pain. Vaccine clinic 2 received a delivery from our manufacturing site located in Norridge Illinois. In order to protect patient safety, we have temporarily halted production of the vaccine until the Pharmalilly interprofessional team can identify how to get to the source of these adverse reactions.
