Draw your ER diagram based on the following narrative about a hospital. Be sure to indicate the various attributes of each entity and relationship, underline the identifier of an entity, and specify the cardinalities and cardinality constraints for each relationship. A hospital depends on four major groups of persons for its continued success: employees, physicians, patients, and volunteers. A small number of persons in the hospital community do not belong to any of these four groups. A particular person may belong to two (or more) of these groups at a given time. For example, a volunteer or employee may also be a patient at the hospital at some point in time. Furthermore, a particular person can have zero or more dependents, and a particular dependent is dependent of only one person. A dependent has attributes: Name, Gender, and Relationship to the person employed at the hospital. A dependent cannot exist without the employed person that s/he is related to. The four groups of people listed above share many common characteristics such as a unique identifier (SSN), Name, Street Address, City, State, Zip_Code, Birth_Date, Phone, and Email. Then there are characteristics that apply to only one of these groups. For example, a hire date (Date_Hired) is recorded for employees only, and Skills are recorded for volunteers in order to place them appropriately. Physicians have pager number (Pager_No) and Specialty (e.g. pediatrics, oncology, etc.). For patients, the hospital records the date of first contact with the hospital (Contact_Date). In addition to the characteristics already mentioned, the hospital records a number of other characteristics about its patients: emergency contact information (EC_Name, EC_Relationship, and EC_Phone), insurance information (Insurance_Name, Policy_No, Group_No, Insurance_Phone), and information about insurance subscriber in case the patient is not the insurance subscriber (Subs_Name, Subs_Relationship, Subs_Address, Subs_Phone). At this hospital, each patient has one (and only one) physician responsible for that patient. A given physician may not be responsible for a patient at any given time or may be responsible for one or more patients. Furthermore, the hospital wants to keep the information on the treatment that the patients receive from their physicians. The only attribute of treatment is the identifier Treatment_ID, which consists of two components: Treatment_No and Treatment_Name. Each physician can perform many treatments to many patients or does not perform any treatment to any patient at all; each treatment can be performed by many physicians to many patients, or it is not performed by any physician to any patient at all; and each patient can receive many treatments from many physicians, or the patient may not receive any treatment from any physician at all. Moreover, the hospital wants to track when a treatment is performed by a physician to a patient (Treatment _StartDate), and the results of the treatment (Results). This two information is tied to a specific physician who performed a specific treatment to a specific patient.
