Each person's individualised plan will outline everyone's roles and responsibilities, the required service outcomes, the person's needs and preferences, the resources that are available and the support strategies that must be followed. A learning plan that addresses the person's specific skill development needs will also be developed, in collaboration with the person, their family, significant others and relevant experts.
