Event monitor with PVCs and one run of NSVT that was accompanied with dizziness. Patient reports history of requiring adenosine x3 for SVT in past. Patient does state that his blood pressure has been low and that he has not been taking the full dose of his BB [beta-blocker]. He also continues to have high anxiety with palpitations and can't tell which comes first. Patient denies any near syncope. He does report a midsternal lightness at times with dyspnea. He underwent loop recorder implantation last month with Dr. Mctavish. Patient reports continuing to feel his heart racing, typically at night without activity. Denies any fevers or chills. Denies any CP, SOB, palpitations, dizziness, or near syncope. Dressing removed from LCW, steri-strips C/D/I. His heart rate appears to stay high no matter what he does, however, pain has recently been resurfacing. Patient notes feeling a lot of anxiety for which he takes Xanax PRN. He notes that he has been cold lately as well and he does not exercise because he is afraid of increased HR. The patient endorses still having palpitations daily, low BP, dizziness and lightheadedness that happens even at rest. Admits to weight gain (15 lbs) despite eating the same (current weight 216 lbs, was 206 lbs a month ago). Has not had thyroid checked in more than a ear. Notes that palpitations are triggered by panic attacks and would subside after taking Xanax but does not want to take Xanax all the time. Had been seen by a psychiatrist but no further treatment was done. The patient works part time as an administrative assistant. It is my opinion that this patient can continue to be treated medically and no surgical intervention is warranted at this time.
