Question:
1. Using an appropriate diagram discuss the risk appetit
2. The case study is a well-known incident that took place in 2011 in Japan and it has been mentioned that this could have been avoided. Propose how the use of risk management could have been prevented?
3. Propose the different techniques that can be used to identify the risks in the case study and justify your response.
Question 2
1. Discuss at least 5 inherent risks that should have been identified during the risk assessment process? Students must clearly specify the risk identifier, risk category, risk description, risk cause and effect.
2. Using the risk register in the previous section, conduct a qualitative risk analysis for these 5 risks by proposing the probability of occurrence and impact for each risk? The risk matrix included in appendix 1 must be used to rate the inherent identified risks and students must discuss the chosen probability and impact. The table below should be used to show your response.
3. Using the risk matrix included in appendix 1, plot the inherent risks on a heat map and discuss what the map means?
Question 3
Using the table below, propose the most appropriate risk responses for each of the risk identified in question 2.1. If any of the risks are not acceptable, then propose sufficient (more than 1 if needed) controls to make them tolerable (acceptable).
3. Discuss how the level of confidence can be illustrated on a risk matrix?
4. With aid of an appropriate diagram, show the impact of the various controls on the inherent risks identified in question 2.1 using the level of confidence methodology?
Question 4
1. Classify the control category for each control discussed in the question 3.1 and justify your response? Tabulate your responses.
2. Propose a Risk Management Framework that could be implemented at the Nuclear Facility?
Identify technological skills in the following case scenario and use the information to complete the table provided after. Dr. Jacobs invited his patient Kerry to take a seat as soon as she entered his office. When she seemed settled, he asked, "How can I help you today?" Kerry noticed that he had retrieved her electronic file and had begun to input information such as the date and time and the complaint she was making. She found herself for a brief moment wondering if he had kept his database updated on all his patients but was interrupted by his other question, "What have been the symptoms since you have stopped taking your medication?" As she spoke he began to make sketches on a notepad. When he was through with his sketching, he quickly typed his notes and asked Kerry a few other questions. He seemed satisfied with his notes as he pointed her to another room where he had various tools and gadgets and an ultrasonic machine. As usual, these resources seemed well kept. From Dr. Jacob's indication of the next stage of the process, Kerry knew immediately that it was time to move from questioning her to gather additional information for her historical profile to validating his preliminary diagnosis with actual tests. What would he find? Not being sure of the answer, Kerry settled with the only decision that seemed plausible for the moment. She could only wait for his diagnosis.
